Documents checklist

What Slovak citizens usually need to bring for the most common requests, based on official Slovak government information:

Slovak passport (application abroad)

  • Book an appointment (many embassies use an electronic booking service) and apply in person (applicants 18+)
  • A valid Slovak ID card or Slovak passport as proof of citizenship – a driving licence is not accepted
  • Without either: first obtain a Slovak citizenship certificate
  • Name changes abroad: the apostilled certificate with Slovak translation
  • Passports are produced in Slovakia; lost passport: emergency travel document for the return to Slovakia
